Forms for Tele behavioral counseling
For first time clients, you will need to complete both the tele behavioral and standard forms prior to the session. Once you complete them, save them to a pdf file. From there, you can email them to markm@myerscounseling.com, fax them to (815)356-7044, or once the session starts, we can file transfer. If the paperwork is not completed, that could result in a delay of the session. Also, please be aware that tele behavioral sessions need to be scheduled directly with your therapist.
These standard intake forms need to be completed along with tele behavioral forms.
